It is twice blest: It blesseth him that gives and him that takes.’ – Portia, William Shakespeare, The Merchant of Venice, Act 4, Scene 1.
This quote was inscribed upon a large rock plaque that was visible daily to all medical residents, including myself. Every day, for three years, I read this quote both at the beginning and the end of my 12 and 24 hour shifts. The quote has now become a personal motto for me during my medical career. Serving patients in need of medical care is definitely a privilege and an honor, but, even more so, a blessing. I wish to form physician-patient relationship with you that will help improve your physical health, mental health, and overall well-being” – Dr. Kortni McCall, D.O.

“Patients seek the aid of physicians for assistance in ending their suffering. Suffering can come in many different forms but for the one experiencing it, it is painful. I hope to center my practice on assisting my patient to end their own suffering. That could involve you taking a medicine prescribed, changing your diet, ending an addiction, understanding more about your disease, adjusting your lifestyle to prevent further illness or even questioning your stressful thoughts. In my experience, physicians can provide sound medical advice but it is up to you, the patient, to take charge of your own physical and mental health.” – Dr. Brooke Uptagrafft –

Once you have added your name and submitted the form on this page, your name will be added to our list to receive an email invitation to begin the new patient enrollment process when we resume accepting new patients.
Please remember to bring your photo ID and insurance card to your new patient appointment so we can scan them into our system. The only paperwork you will need to complete at the office will be your HIPAA form and a medical records request if we need to receive any records from prior physicians.
