This quote was inscribed upon a large rock plaque that was visible daily to all medical residents, including myself.  Every day, for three years, I read this quote both at the beginning and the end of my 12 and 24 hour shifts.  The quote has now become a personal motto for me during my medical career.  Serving patients in need of medical care is definitely a privilege and an honor, but, even more so, a blessing.  I wish to form physician-patient relationship with you that will help improve your physical health, mental health, and overall well-being” – Dr. Kortni McCall, D.O.

About Dr. McCall

Dr. McCall specializes in internal medicine as well as hospital medicine, caring for patients here at Innova Primary Care as well as patients at Crestwood Medical Center.  She enjoys reading on a variety of topics, listening to an eclectic range of musical tastes, as well as traveling the world. She teaches medical students who are in the process of becoming physicians themselves.  She participates in medical mission trips and is especially interested in sea turtle conservation efforts.

Dr. McCall and her healthcare team personally work with patients on the following:

  • Handling the complicated aftercare of patients having suffered heart attacks, strokes, etc.

About

Amy is originally from Lubbock, TX and graduated in 2014 with her Masters of Science in Nursing from the University of Alabama in Huntsville as a Family Nurse Practitioner. Prior to becoming a CRNP, Amy served eleven years as an Aerospace Medical Service Technician in the United States Air Force before obtaining her Bachelors of Science in Nursing in 2011.  She worked for several years in a local Family Practice office as a registered nurse and has taught as a nursing clinical instructor at the University of Alabama in Huntsville.  Amy is married to her husband Robert who is a Cybersecurity Engineer on Redstone Arsenal.  They have three daughters. Outside of her time serving our patients, Amy enjoys watching football and soccer, reading, and spending time with her family and friends.

    Dr. Brooke Uptagrafft grew up here in Huntsville and returned home to her home in 2011 to open her primary care practice. Dr Uptagrafft graduated from Grissom High School, then Mississippi State University and received her medical degree from The University of Alabama in Birmingham (UAB). She completed her residency at St Vincent’s East specializing in Family Medicine. She operates Innova Primary Care alongside her team of Doctors and Certified Registered Nurse Practitioners (CRNP) with the goal of mending patients and mending standards in our community.

      About

      David Uptagrafft is the CFO and Director of Healthcare Experience at Innova Primary Care where he oversees financial performance as well as strategic quality and patient experience programs for the clinic. Innova’s mission is to mend people and standards for healthcare in our region through the practice of individualized medicine, the development of trusted patient relationships and the aggressive adoption of new technologies to improve population health management. Innova Primary Care has been recognized for innovative leadership and quality by Blue Cross Blue Shield of Alabama, The University of Alabama in Huntsville, NCQA, the Huntsville Madison County Chamber of Commerce and the Consortium for Southeastern Hypertension Control (COSEHC).

      Previously, David spent seven years providing software product management and business consulting services with SunGard Financial Systems (now Fidelity Investment Services). His emphasis during this time was on building distributed international software development and support teams for cloud-based wealth management systems and integration of third party software solutions as companion offerings to products. During his time at SunGard David completed his Project Management Professional certification.

      David also served as Vice President and Manager of Online Services for Hancock Fabrics Inc, participating in the development and launch of an ecommerce website recognized by Internet Retailer Magazine as one of the top 500 sites nationally at the time. Additionally, David designed and oversaw the development of the integrated distribution center inventory system for franchisees and customers.

      Brittney is originally from Scottsboro, Alabama but moved closer to Huntsville several years ago.  Brittney joined the Innova team in 2015 and has led the Innova team forward into the Chateau Drive location. As Office Manager, Brittney is responsible for oversight of the Innova team as well as Innova guests. Brittney frequently works off-stage on projects, but she does still serve guests in clinic as well. On a calm day, you might catch her walking back from the Starbucks next door.

      Megan Ladner, CRNP

      About

      Megan is from Huntsville, AL and graduated from the University of Alabama in Huntsville with her Bachelors of Science in Nursing in 2013 and her Masters of Science in Nursing in 2019. Prior to becoming a CRNP, she worked for 6 years as a critical care RN. She has worked in various ICUs including Surgical Trauma, Neuro, Cardiac, and Medical with multiple years of travel nursing experience. In 2018 Megan married her husband Andy, a Neuro-ICU RN. Together they enjoy all things Disney, rafting in Colorado, traveling to new places, and spending time with their dogs, Bo and Poppy.

      Kelley Rogers, CRNP

      About

      Kelley considers Huntsville, Alabama to be her hometown. Kelley received bachelor degrees in Biology and Nursing before working at Huntsville Hospital in the Surgical Trauma ICU as a Registered Nurse. Kelley received her Master of Science in Nursing as a Family Nurse Practitioner in 2015 from the University of Alabama in Huntsville. She and her husband Michael, a chemist, were married in 2012. Together they enjoy the outdoors, family, friends and their dogs.

      Medical Interests:

      Kelly enjoys educating her patients on how to implement a healthy diet and exercise to assist in disease prevention.  Her other interests include working with pediatric patients as well as providing patient education.

      Haley is originally from Hazel Green and worked as a Registered Nurse in the Emergency Department at Crestwood Medical Center for 3 years. Haley was a Graduate Teaching Assistant for UAH’s College of Nursing while obtaining her graduate degree. Haley is soon-to-be married to Jake, who works in Business Management. Haley enjoys exercising, traveling, and spending time with family, friends, and her kitty cat Charlie.

      Angelia Hart, CRNP

      About

      Angie is originally from Butler, Alabama and graduated in 2013 with a Master’s of Science as a Clinical Nurse Specialist from the University of Alabama in Huntsville. In 2015 she received her Post Masters Certificate as a Family Nurse Practitioner. Prior to becoming a CRNP she worked as a nurse in the Surgical Trauma ICU at Huntsville Hospital and taught as a nursing clinical Instructor at the University of Alabama in Huntsville.  Angie is married to Michael, a Software Engineer on Redstone Arsenal. They are parents to 5 children and enjoy spending time together as a family.

      Medical Interests:

      Angie’s interests include managing and educating patients on diabetes care.  She also enjoys caring for special needs children and adults as well as providing geriatric care.

      What to Expect Next

      Once you have added your name and submitted the form on this page, your name will be added to our list to receive an email invitation to begin the new patient enrollment process when we resume accepting new patients.

      1. On the new patient pre-registration date, you will receive an email inviting you to pre-register to be a new patient at Innova Primary Care. Once you have completed the information requested, we are typically able to process your pre-registration within 3 business days.
      2. You will receive a “Welcome New Patient” email to the email address you have submitted on your pre-registration with instructions to complete your medical history questionnaire on your patient portal.
      3. Once we receive your medical history questionnaire, you will receive a portal message, typically within 1 business day, with a link to click on to schedule a good day and time for us to call you to finish your new patient registration. During the phone call we will get your insurance information and go over our Innova Relationship Agreement and put a payment method on file for you.
      4. Welcome to Innova Primary Care! It’s time to schedule your new patient appointment, we look forward to meeting you face to face.

       

      Please remember to bring your photo ID and insurance card to your new patient appointment so we can scan them into our system. The only paperwork you will need to complete at the office will be your HIPAA form and a medical records request if we need to receive any records from prior physicians.
