FAQs

What Should I expect on my first appointment?

Now that you have been accepted as a patient, we would like you to know what you need for your appointment and what to expect.

When you come into Innova Primary Care for the first time you will be greeted by our Guest Receptionist, who will be happy to help you. You will be asked for your DL and Insurance cards to scan into your chart. Once that is complete, we will show you how to check in on our kiosk and pay your copay through the kiosk all while sitting comfortably in the lobby. We collect all copay’s at time of service. If you are unable to make you copay, please let our office know and we will be glad to reschedule you. We will also have you complete a HIPPA form which is required once a year.

Now that you have checked in, we have a coffee station in our lobby for you to enjoy while you wait. Our wait time is typically between 5-10 minutes.

After completion of your new patient appointment, we are here to meet your medical needs as they arise. We also provide Same Day Sick Appointments.

How Does New Patient Pre-Registration Work?

On our homepage you you will find a “New Patient Application” link to click on that will bring you to our accepted and non-accepted insurances along with a brief explanation on controlled medications. You will then click the “Pre-registration” link and complete the required fields for each person requesting to be a patient.

Once you submit your new patient registration, you will receive a “Welcome New Patient” email to the email address you have submitted on your pre-registration. Please allow up to 1 business day to receive your “Welcome New Patient” email. You will also receive a separate email from us with your portal information for accessing our patient portal. The next step will be to complete your medical history questionnaire on the patient portal. Once we receive your medical history questionnaire, you will receive a portal message within 1 business day asking for your insurance information and your best days and times so we can schedule your new patient appointment. After submitting all the required insurance information and availability, you will then receive a portal message with your new patient appointment date and time.

Please remember to bring your photo ID and insurance card to your new patient appointment so we can scan them into our system. The only paperwork you will need to complete at the office will be your HIPAA form. Otherwise, you are all set for your new patient appointment.

If you have any further questions or concerns about our new patient process, please feel free to contact us.
